Download and Complete Your Patient Forms

Step 1: Downloading the Forms

• Choose the File Format: Select the preferred file format for your forms (usually PDF). Click on the download link to initiate the download process.
• Save the Forms: Once the download is complete, save the forms to a location on your computer or device that is easily accessible.

Step 2: Completing the Forms

• Print and Fill Out (Option 1): If you prefer handwritten forms, print the downloaded PDF, fill in the required information, and bring the completed forms with you to your appointment.
• Fill Out Electronically (Option 2): If you’d rather complete the forms electronically, open the downloaded PDF using a word processing application such as Microsoft Word, Pages, or another compatible program.
• Save Your Changes: After filling out the forms electronically, save the document with your changes to your computer or device.

Step 3: Submitting the Completed Forms

• In-Person Submission: If you chose Option 1, bring the printed and completed forms with you to the dental practice on the day of your appointment.
• Email Submission: If you opted for electronic completion, attach the saved PDF document to an email. Send it to [insert practice email address]. Please ensure the subject line includes your full name and appointment date.
